Life Without Limits Inc. is seeking a FULL TIME Day Habilitation Manager. We are an active adult day program that provides daily living skills and community outings to IDD adults. Shifts is Monday -Friday 7:30am-4:00pm

Ensure the implementation of all facility based and community curriculum which motivates and enhances the lives of the people served. Provide leadership, supervision and support to staff and participants to maximize human dignity, community integration, and opportunities for self-direction. The manager will be the bridge between individuals served and external stakeholders by being the primary point of contact.

Requirements

  • Oversee daily operations to assigned area in accordance with the agency’s policies, procedures, and applicable regulations.
  •  Supervise, evaluate, and train Day Habilitation and Community Based Day Services (LWOL) staff.
  • Work collaboratively with team members demonstrating a positive and supportive attitude to all staff and participants both during interventions and activities as well as all other assigned duties throughout the day.
  • Be active within the program rooms throughout the day to ensure that scheduled curriculum, individualized behavioral plans, DHSPs, ISPs, feeding protocols and other health and safety related protocols are being implemented.
  • Identify issues or concerns regarding participants’ clinical needs and communicate and advocate with appropriate parties.
  • In conjunction with Clinical Department, monitor the implementation of established curriculum in conjunction with developmental skills areas. Schedule and ensure implementation of community-based activities that promote personal choice, community inclusion, dignity and respect, while enhancing personal and vocational skills.
  • Ensure the implementation of Day Habilitation Support Plans, Behavior treatment plans, Individual Support Plans, and other service plans which enable participants to increase their self-sufficiency and ability to fully participate in services.
  • Oversee the ISP process for assigned program participants in coordination with the Executive Assistant  of Day Services. Facilitate ISP meetings and secure all necessary consents.
  • Perform  Manager duties including progress note completion, assessments, support plans, annual severity profiles, and community activity documentation.
  • Participate in the intake, admission, transfer, and discharge process for participants. Provide communication regarding same with direct reports. Complete documentation and activities related to process.
  • Know and understand profiles and histories of individuals served to ensure that appropriate goals are developed and implemented for individuals.
  • Ensure compliance with DDD regulations, and Incompass policies and procedures. Provide support to staff on incident and accident injury reports related to participants as necessary.

  • Must maintain valid Driver’s license and clean driving record.
  • 3-5 years of related experience working with individuals with disabilities.
  • Previous experience managing/working in a unionized workplace highly desirable.
  • Must be able to pass Background Check and Drug Test
